Appointments are scheduled and confirmed during a free 5-10 minute telephone consultation. Based on evaluation and medical recommendations visit may include -- Specialist referrals -- Blood draws, COVID/Flu swabs, urine testing -- EKG's **IV Infusion can be added to visit at additional cost**

Cancellation Policy

Based on the type of appointment scheduled Infinity Family Health NP Consultant PLLC may require a debit or credit card be placed on file for charges related to your scheduled visit. This card will be charged on the day of your appointment for any co-payments or fees due. If your insurance determines you are responsible for additional charges a statement will be emailed and mailed to your home. It is the responsibility of the client to make sure Infinity Family Health NP Consultant PLLC has your correct email and home address for statements/bills to be delivered. If you do not agree to the balance on the statement you must call our office at 845-535-1714 to contest the balance within 7 days. All self-pay appointments must be paid in full on the day of your appointment. All patients are required to email a copy of the front and back of their insurance and state ID card to infinityfamilyhealth23@gmail.com within 48 hours of a scheduled appointment to avoid appointment cancellation. In the case of virtual visits patients are responsible for having the app downloaded correctly and be in an area that has sufficient Internet coverage or cell coverage to allow for the visit. If the patient does not have the app downloaded correctly or does not have sufficient Internet or cell coverage to complete the visit this will be considered a missed appointment and the patient will be required to pay a no-show fee to re-book subsequent appointments. We strongly encourage all patients to test the app prior to their appointment time to avoid any technical difficulties that may result in a no show and subsequent fees.
