What are the out of pocket fees for medical concierge visits?

Health insurance coverage will be pre-approved prior to every visit.

Out-of-Pocket Payers: fees are individualized based on patient need, and are discussed during the free 5-minute telephone consultation prior to scheduling your appointment. Home visit fees will vary by the length of the visit and the level of care required during the visit and will always include travel time required.

How do I pay for my visit?

Infinity Family Health NP Consultant's policy requires all patients with co-pays, co-insurances and out of pocket payers to signed a credit or debit card authorization agreement, prior to each patient visit. Telehealth Appointments: Your credit/debit card will NOT be charged until tele-health visits has been completed, and your card will NOT be stored for future visits. A new signed credit/debit card agreement will be required prior to each visit. Home Visits: A deposit of up to 50% may be required to schedule your appointment. Balances will not be charged until home visits are completed. A full refund will be issued when appointments are canceled 24 or more hours prior to appointment time.

 

IV Infusion Visits: deposit of up to 75% may be required to schedule your appointment. Balances will not be charged until infusion visits are completed.
